Appointments

For appointments please call 728-6101 or 1-800-823-2663. Please tell the receptionist the reason for your visit so ample treatment time is arranged.

New Patients

If you have not been in our office or seen one of the physicians, you may want to facilitate the collection of your personal information by downloading and completing the appropriate forms. All patients will need to fill out the Patient Information Form and the Patient History Form. Patients with injuries covered by Workers' Compensation will need to fill out the New Patient Workers Compensation Form. If you have a knee or shoulder problem, you will need a Knee or Shoulder Evaluation Form. Please give the completed forms to the receptionist when you arrive at the office.

Established Patients

If you have not been in our office in the past year, you may want to facilitate the collection of your updated personal information by downloading and completing the appropriate forms. Please give the completed forms to the receptionists when you arrive at the office. You may also click on the online links to schedule follow up appointments, check appointment times, or cancel appointments (24-hour notice required.)
